In a significant breakthrough, researchers from Japan's Ryukoku University and Osaka Medical University have successfully developed a drive mechanism capable of controlling the movement of capsule endoscopes in laboratory settings. This innovation has now progressed to successful in vivo trials, marking a major step forward in medical technology.

The new drive system consists of two main components: first, a unit that generates and controls an alternating magnetic field, and second, a micro-mechanical device equipped with a small magnet and a "tail." When placed within the magnetic field generated by the first component, the magnet vibrates, causing the tail to move in a manner similar to how a fish swims. By adjusting the AC waveform of the magnetic field generator, the speed and direction of the micro-machine can be precisely controlled from the outside.

This technology is integrated into capsule endoscopes, allowing external control over their movement. The in vivo experiments used a small intestine capsule endoscope produced by Israeli company Given Imaging. The design allows for easy adaptation to various capsule models simply by adjusting the size of the mounting socket.

The Given Imaging capsule has an outer diameter of 11 mm and a length of 26 mm. After installing the micro-mechanical unit, the modified capsule used in the live experiment measured 14 mm in diameter and 48 mm in length. For optimal movement within the stomach, the capsule requires prior ingestion of water to float and navigate effectively.

Experiments were conducted on dogs, confirming three key findings:

  1. The magnetic field does not interfere with image capture or transmission, ensuring clear and uninterrupted display of the video feed.
  2. The capsule's movement can be remotely controlled, allowing for precise adjustment of speed and direction.
  3. The capsule was able to locate and photograph four pre-placed hemostatic clips in the stomach, demonstrating its potential for targeted imaging.

Capsule endoscopy was first introduced in 2001 and gained regulatory approval in Japan in April 2007 for Given Imaging’s small intestine model. Olympus Medical Systems also received approval in September 2008. However, current capsule endoscopes cannot be externally controlled and rely on natural peristalsis for movement.
