Application of multi-slice spiral CT angiography in arteritis

Ma Xiangxing, Zhang Wei, Ma Xiaofeng, Wang Qing, et al. Objective: The study aimed to evaluate the clinical significance of multi-slice spiral CT (MSCT) angiography in diagnosing arteritis. Methods: A total of 14 patients with clinically suspected aortitis underwent MSCT angiography. Scanning was performed from the cervical vertebrae down to the pubic symphysis. Image post-processing techniques included multiplanar reformation, maximum intensity projection, volume rendering, virtual endoscopy, and advanced vascular analysis. These methods were used to visualize the location, extent, and severity of affected vessels. Results: CT findings revealed varying degrees of arterial wall thickening, luminal stenosis, or occlusion. Stenosis was often centripetal, with some cases showing partial dilation. Eight patients had stenosis-obstructive type, while six had mixed types. The combination of reconstructed and axial images provided clear visualization of the degree of stenosis, occlusion, and collateral vessel formation. According to the Lupi-Herrea classification, five were type I, two type II, six type III, and one type IV. CT results correlated well with clinical symptoms. Conclusion: MSCT angiography effectively evaluates both the luminal and mural changes in aortitis. Assessing wall thickness is particularly valuable for early diagnosis, treatment planning, and prognosis improvement. It can serve as the primary imaging modality for this condition.
