New strategies for preventing and treating myocardial remodeling in high-salt obesity

The basic and clinical studies on high-salt and obesity-related myocardial remodeling completed by the Chengdu General Hospital of Xining Lianqin Support Center (formerly Chengdu Military Region General Hospital) and the Northern Sichuan Medical University for many years found that exercise, neurohormones, melatonin, and renal sympathetic Nerve ablation plays an important role in high salt fat-related myocardial remodeling. The current consensus is that high salt and obesity are one of the important factors causing myocardial remodeling, and myocardial remodeling is a key link in the development of heart failure. However, the clinical effect of using limited salt and lipid regulation to reduce myocardial remodeling is limited. This study found that heat shock protein (Hsp70) is a key molecule in the remodeling of high-salt fat obese myocardium. Exercise can make Hsp70 increase significantly and play a protective role in the heart. The male Hsp70 increased significantly, suggesting that men with high salt and obesity should choose long-term exercise to enhance the tolerance to myocardial ischemia. This provides theoretical support for the individual control of myocardial remodeling.

The study also revealed a new mechanism of endocrine regulation of high-salt obesity-related myocardial remodeling. Loss of melatonin secretion and rhythm disorder in patients with high-salt obesity can improve sleep quality and play a role in myocardial protection by activating receptors on cardiomyocytes. High-salt obesity results in increased sympathetic activity and causes myocardial remodeling, whereas renal sympathetic ablation can reduce or inhibit myocardial remodeling.

Yang Yongjian, president of the Chengdu General Hospital, said that with a small guide catheter inside the power wire, the renal sympathetic nerve can be radiofrequency ablation, thereby reducing its excitement.

Clinically, after undergoing renal sympathetic nerve ablation in patients with multiple high-salt, obese, myocardial remodeling, cardiac ultrasound revealed a significant reduction in left ventricular septal and posterior wall hypertrophy. “This minor operation is not complicated and it is not dangerous. As of now, the hospital has done many cases of renal sympathetic nerve ablation, and more than 70% of patients have their symptoms relieved. But not every high-salt, obese patient is suitable.
